I installed KeeperFX and all the goodies and it works like a charm. Out of curiosity I turned on fraps and it shows consistent 20fps. Was DK1 simply designed to run at 20fps or is it some limitation I'm not aware of ?

While I wouldn't trust the results of FRAPS, (last release was nearly a decade ago) that does sound about correct.

After all, the game was pushing a graphical edge at the time.

Yes, and FPS is linked to game speed. With KeeperFX you can easily set the fps higher, but then everything goes faster too.
